目的探讨彩超引导下Encor微创旋切术在触诊阴性的乳腺微小肿块切除术中的应用价值。方法对189例女性患者,在彩超实时引导下,应用Encor微创旋切系统,完整切除413个触诊阴性乳腺微小肿块。结果 189例患者均顺利完成了手术,术中疼痛可忍,VAS评分均在3分以下,术中无1例皮肤误切。术后均无感染发生、乳房变形和明显瘢痕形成,患者满意,术后3月复查彩超,无1例肿物残留。在≤3个肿块的107例患者中,术中出血、术后血肿、皮下淤斑发生率分别为5.61%(6/107)、11.21%(12/107)和22.43%(21/107);在>3个肿块的82例患者中,术中出血、术后血肿、皮下淤斑发生率分别为8.54%(7/82)、19.51%(16/82)和32.92%(27/82),差别有统计学意义(P<0.05)。所有血肿、皮下淤斑随访3月均自行吸收消失。结论彩超引导下触诊阴性乳腺微小肿块Encor微创旋切术,具有定位准确、切除完整、瘢痕小、乳房无变形等优点,是触诊阴性乳腺微小肿块患者的首选手术方法之一。
Objective To investigate the application value of ultrasound-guided Encor system in minimally invasive surgery for nonpalpable breast tiny lesion resection. Method Four hundred and thirteen nonpalpable breast tiny lesions of 189 female patients were excised by the application of high-frequency color Doppler ultrasound guided real-time monitoring Encor vacuum assisted minimally invasive cutting system in minimally invasive surgery. Results All of the nonpalpable breasttiny lesions of 189 case patients were completely resected. Intraoperative pain can be managed,VAS score was below 3 points in all patients,and no skin miscut was reported. There was no postoperative infection,breast deformation,or obvious scar formation, and all patients indicated satisfaction. High-frequency color Doppler ultrasound 3 months after showed no mass residue. The incidence rate of intraoperative bleeding,postoperative hematoma,subcutaneous ecchymosis were 5. 61%,11. 21%,and 5. 61%,respectively in the group with ≤3 tiny lesions in 107 patients,and 8. 54%, 19. 51%,and 32. 92%,respectively in the group with > 3 tiny lesions in 82 patients,the differences were all statistically significant ( all P < 0. 05) between the two groups. All the hematoma and subcutaneous ecchymosis were absorbed and disappeared during the follow-up of 3 months. Conclusion High frequency ultrasound-guided Encor system in minimally invasive surgery has many superiorities such as accurate locating, high complete resection rate,smaller scars,no breast deformation,etc. It can be used as preferred surgical approach for the benign nonpalpable breast tiny lesions excision.
