右美托咪定对单肺通气患者肺组织水通道蛋白-1及肺功能的影响▲
Effects of dexmedetomidine on the lung tissue aquaporin-1 and lung function in patients undergoing one-lung ventilation

目的探讨右美托咪定对单肺通气(OLV)患者肺组织水通道蛋白-1(AQP1)及肺功能的影响。方法选择择期行胸腔镜下肺癌根治术治疗的患者40例,按照随机数字表法将其分为对照组(C组)和右美托咪定组(DEX组),每组20例。两组均行OLV,DEX组在麻醉诱导后静脉输注负荷剂量为0.5 μg/(kg·h)的右美托咪定(给药时间大于10 min),继之以0.5 μg/(kg·h)的速度维持至手术结束前30 min,C组给予等容量的生理盐水。分别于给药前(T0)、OLV 60 min (T1)、OLV 90 min (T2)、OLV 120 min (T3)、恢复双肺通气30 min (T4)时采集两组桡动脉血样进行血气分析,计算呼吸指数(RI)、氧合指数(OI)和肺顺应性(CL);于OLV前即刻、肺叶离体时取肺组织,测定肺组织AQP l的表达情况。结果T0时,两组患者的RI、OI、CL比较,差异均无统计学意义(均P>0.05)。与T0比较,两组T1~4时RI升高,OI及CL降低(均P<0.05);与C组比较,DEX组T1~4时OI及T2~4时CL均较高,T1~4时RI较低(均P<0.05)。C组肺叶离体时肺组织AQP1表达均低于OLV前和DEX组(均P<0.05),而DEX组肺叶离体时和OLV前的AQP1表达差异无统计学意义(P>0.05)。结论右美托咪定可改善OLV期间的氧合和呼吸动力学,改善肺功能,其机制可能与上调肺组织AQP1的表达有关。
ObjectiveTo explore the effects of dexmedetomidine on the lung tissue aquaporin-1 (AQP1) and lung function in patients undergoing one-lung ventilation (OLV). MethodsA total of 40 patients scheduled for thoracoscopic radical resection of lung cancer were selected and randomly divided into control group (C group) and dexmedetomidine group (DEX group), with 20 cases in each group. Both groups were performed OLV. After the anesthesia induction, the DEX group received an intravenous infusion of a loading dose of 0.5 μg/(kg·h) dexmedetomidine (administration time >10 minutes), followed by a 0.5 μg/(kg·h) infusion velocity which was maintained until 30 minutes before the end of the surgery, while the C group was given the same amount of normal saline. Before administration (T0), OLV 60 minutes (T1), OLV 90 minutes (T2), OLV 120 minutes (T3), and 30 minutes after recovering to double-lung ventilation (T4), the radial artery blood sample was obtained for blood gas analysis; the respiratory index (RI), oxygenation index (OI), and lung compliance (CL) were calculated. Right before OLV and when the pulmonary lobe left the body, the lung tissues were obtained to detect the expression of AQP1 in the lung. ResultsAt T0, the RI, OI, and CL had no statistically significant difference between the two groups (all P>0.05). Compared with T0, the two groups had higher RI and lower OI and CL from T1 to T4 (all P<0.05). As compared with the C group, the DEX group had higher OI but lower RI from T1 to T4, and higher CL from T2 to T4 (all P<0.05). When the pulmonary lobe left the body, the C group had lower AQP1 expression than that before OLV and the DEX group (all P<0.05); however, there was no statistically significant difference in the AQP1 expression of the DEX group when the pulmonary lobe left the body and before OLV (P>0.05). ConclusionDexmedetomidine can improve oxygenation, respiratory mechanics, and lung function during OLV; its mechanism may be related to the up-regulation of AQP1 expression in the lung tissues.
