目的探讨牙周组织再生术(GTR)联合口腔正畸治疗牙周炎患者的治疗效果。方法选择接受手术治疗的牙周炎患者48例,将其随机分为A组(24例,患牙36颗)和B组(24例,患牙37颗),A组患者行GTR联合口腔正畸治疗,B组患者行单纯GTR治疗。观察并记录术前及术后12个月两组患牙菌斑指数(PLI)、牙龈指数(GI)、龈沟出血指数(SBI)、牙周平均附着(CAL)、牙周探诊深度(PD)、患牙松动率及咬合接触时间(OT)变化情况,以及术后12个月两组患者的临床疗效和并发症发生情况。结果术后12个月,两组患牙PLI、GI、SBI、CAL、PD及患牙松动率均较术前显著降低,且A组上述指标明显低于B组(均P<0.05);两组OT较术前显著延长,且A组OT显著长于B组(均P<0.05)。术后12个月,A组疗效明显优于B组(P<0.05),但两组总有效率比较,差异无统计学意义(P>0.05)。两组患者术后12个月的牙龈组织水肿、切口裂开、牙根敏感等并发症发生率比较差异无统计学意义(均P>0.05)。结论GTR联合口腔正畸治疗牙周炎效果突出,能明显改善患者牙周情况。
ObjectiveTo investigate the therapeutic effect of guided tissue regeneration (GTR) combined with orthodontics in the treatment of patients with periodontitis. MethodsA total of 48 patients with periodontitis who received surgical treatment were selected, and they were randomly divided into group A (n=24, 36 teeth) and group B (n=24, 37 teeth). Patients in group A were treated with GTR combined with orthodontics, and group B was treated with simple GTR. The changes of plaque index (PLI), gingival index (GI), sulcus bleeding index (SBI), clinical attachment loss (CAL), probing depth (PD), teeth mobility and occlusion time (OT) before and 12 months after operation, as well as the clinical efficacy and complications 12 months after operation were observed and recorded in both groups. ResultsTwelve months after operation, PLI, GI, SBI, CAL and PD of teeth and teeth mobility were significantly lower than those before operation in both groups, and the above indexes in group A were significantly lower than those in group B (all P<0.05); the OT was significantly prolonged than that before operation in both groups, which was significantly longer in group A than in group B (all P<0.05). Twelve months after operation, the therapeutic effect of group A was significantly superior to that of group B (P<0.05), but there was no statistically significant difference in the total effective rate between the two groups (P>0.05). There were no statistically significant differences in the incidence of complications of gingival tissue edema, incision dehiscence and root sensitivity between the two groups 12 months after operation (all P>0.05). ConclusionThe effect of GTR combined with orthodontics in the treatment of periodontitis is prominent, which can significantly improve the periodontal condition of patients.
