数字化种植牙外科导板辅助种植体植入术的效果评价▲

目的评价数字化种植牙外科导板辅助种植体植入术的临床效果。方法选取20例需行种植牙修复的牙列缺损患者,利用Segma Guide种植手术设计软件,设计制作牙支持式数字化种植外科导板,经导板辅助植入35枚种植体。术后即刻拍摄锥形束CT(CBCT),将术后CBCT图像导入Segma Guide软件,并与术前CBCT数据进行配准重合,利用软件自带功能测量实际种植体位置与术前设计种植体位置的偏差距离和角度,包括顶部偏差、底部偏差、深度偏差和角度偏差,并对上、下颌的偏差值进行比较。结果35枚种植体术后实际位置与术前设计位置的顶部偏差、底部偏差、深度偏差分别为(1.06±0.52)mm、(1.72±0.64)mm、(0.87±0.28)mm,角度偏差为(3.37±1.27)°。数字化种植外科导板引导种植体植入后,上、下颌种植体模拟位置与实际位置的顶部偏差、底部偏差和角度偏差比较,差异有统计学意义(均P<0.05),但深度偏差的差异无统计学意义(P>0.05)。结论Segma Guide软件设计制作的牙支持式种植外科导板可获得较高的临床三维精度,可用于牙列缺损患者的种植体植入术。
ObjectiveTo evaluate the clinical effect of implant implantation assisted by digital dental implant surgical guide plate. MethodsA total of 20 patients with dentition defects who needed dental implants were selected. Using Sigma Guide implant surgery design software, digital implant surgical guide plates with tooth-borne were designed and manufactured, and 35 implants were implanted with guide plates assistance. Cone-beam computed tomography (CBCT) was taken immediately after the surgery. The CBCT images were imported into Sigma Guide software, and registration and superposition were conducted with preoperative CBCT data. The deviation distance and angle between the actual implant position and preoperative designed implant position were measured with the built-in function in the software, including top deviation, bottom deviation, depth deviation and angle deviation, and the deviation values of the upper and lower jaw were compared. ResultsThe top deviation, bottom deviation and depth deviation between the postoperative actual position and the preoperative designed position of the 35 implants were (1.06±0.52) mm, (1.72±0.64) mm and (0.87±0.28) mm respectively, and the angle deviation was (3.37±1.27) °. After implant implantation guided by the digital implant surgical guide plate, there were statistically significant differences in the top deviation, bottom deviation and angle deviation between the simulated position and the actual position of the maxillary and mandibular implants (all P<0.05), but there was no statistically significant difference in the depth deviation (P>0.05). ConclusionThe tooth-borne implant surgical guide plate designed and manufactured by Segma Guide software can obtain high clinical three-dimensional accuracy, and can be used for implant implantation in patients with dentition defects.
