目的观察玻璃体切割术治疗眼内异物合并视网膜裂孔的临床效果。方法对24例(24眼)眼内异物合并视网膜裂孔患者行玻璃体切割术,取出异物、激光封闭视网膜裂孔、重水气液交换复位脱离视网膜,并行硅油填充。观察术后异物取出情况、视网膜脱离复位、并发症情况,以及手术前后视力变化。结果24例患者Ⅰ期手术异物取出成功率为100%,患者术后视力较前明显提高(P<0.05)。3例(3眼,12.5%)因合并黄斑裂孔,视力无提高;余21例(21眼,87.5%)均有不同程度视力提高。16例视网膜脱离者中14例(14眼,87.5%)成功复位,2例(2眼,12.5%)取油后复发视网膜脱离,再次手术后成功复位。结论玻璃体切除手术是治疗眼内异物合并视网膜裂孔的有效方法,其并发症少,视功能恢复良好。
ObjectiveTo observe the clinical effect of vitrectomy in the treatment of intraocular foreign body complicated with retinal hole. MethodsTwenty four patients (24 eyes) with intraocular foreign bodies complicated with retinal hole were performed with vitrectomy, foreign body extracting, retinal hole closing by laser, retinal detachment resetting by heavy water gas-fluid exchange, and silicone oil tamponade. The postoperative conditions of foreign body extraction, retinal detachment restoration and complications, as well as changes of vision before and after operation were observed. ResultsTwenty-four patients obtained a 100% success rate of foreign body extracting at the first stage of operation, and their postoperative vision significantly improved (P<0.05). The vision did not improved due to the complication of macular hole in 3 cases (3 eyes, 12.5%), but it improved to varying degrees in the other 21 cases (21 eyes, 87.5%). Of the 16 cases of retinal detachment, fourteen (14 eyes, 87.5%) were successfully reset, 2 cases (2 eyes, 12.5%) had their retinal detachment recurred after oil extraction, but were successfully reset after re-operation. ConclusionVitrectomy is an effective method for intraocular foreign bodies complicated with retinal hole, with less complications and better visual function recovery.
