目的比较手动角膜曲率计、光学相干生物测量仪(IOLMaster)、VERION数字导航系统三种仪器测量角膜曲率的重复性及一致性。方法对39例(78眼)白内障患者分别使用上述三种仪器进行角膜生物学测量,记录陡峭轴角膜曲率(Ks)、平坦轴角膜曲率(Kf)、散光轴位,比较三种不同仪器检测结果的重复性和一致性。结果三种不同仪器Kf的2.77 Sw均≤0.19 D,CVw均≤0.16%,ICC均≥0.91;Ks的2.77 Sw均≤0.25 D,CVw均≤0.21%,ICC均≥0.90。不同仪器间的两两比较显示,右眼Kf、左眼Kf,右眼Ks、左眼Ks差值的平均值最大的分别是0.27D、0.24 D、0.14 D、0.16 D。三种不同仪器测量的左、右眼Kf值两两比较时,除手动角膜曲率-VERION差异无统计学意义(右眼P=0.26,左眼P=0.44),其余差异均具有统计学意义。三种不同仪器测量的左、右眼Ks值两两比较时,除手动角膜曲率-VERION差异无统计学意义(右眼P=0.84,左眼P=0.89),其余差异均具有统计学意义。各仪器间的95%LoA均小于0.5 D,一致性区间较窄。结论三种仪器测量角膜曲率的重复性好,精度高,具有较好的一致性。
ObjectiveTo compare the repeatability and consistency among three instruments, namely, manual keratometer, optical coherence biometer (IOLMaster) and VERION digital navigation system, for measuring corneal curvature. MethodsThe three instrument mentioned above were separately applied to 39 patients (78 eyes) with cataract for corneal biometry, and their steep keratometry (Ks), flat keratometry (Kf) and astigmatic axis were recorded. The three different instruments were compared in terms of repeatability and consistency of their detection results. ResultsThe 2.77 within-subject standard deviation (Sw) and within-subject coefficient of variation (CVw) for Kf were all less than or equal to 0.19 D and 0.16%, respectively, among the three different instruments, and intraclass correlation coefficient (ICC) greater than or equal to 0.91; the 2.77 Sw and CVw for Ks were all less than or equal to 0.25 D and 0.21%, respectively, and ICC greater than or equal to 0.90. The pairwise comparison between different instruments revealed that the maximum average values of Kf difference in the right eye, Kf difference in the left eye, Ks difference in the right eye, and Ks difference in the left eye were 0.27 D, 0.24 D, 0.14 D and 0.16 D, respectively. After a pairwise comparison, the Kf values of left and right eyes measured by the three different instruments, except for manual corneal curvatures measured by VERION system (right eye P=0.26, left eye P=0.44), showed statistically significant differences. After a pairwise comparison, the Ks values of left and right eyes measured by the three different instruments, except for manual corneal curvatures measured by VERION system (right eye P=0.84, left eye P=0.89), showed statistically significant differences. 95% limits of agreement (LoA) for each instrument was less than 0.5 D, with a narrow consistency range. ConclusionThe three instruments have a good repeatability and a high accuracy in measuring corneal curvature, achieving a favorable consistency.
