目的评价乳晕入路腔镜甲状腺切除术对机体的创伤程度。方法选择60例甲状腺良性肿瘤患者作为研究对象,其中接受乳晕入路腔镜手术治疗的30例为观察组,接受传统开放手术治疗的30例为对照组。统计分析两组患者术前1 d及术后24 h、48 h、72 h血清皮质醇(CORT)、肿瘤坏死因子α(TNF-α)、白细胞介素6(IL-6)、C-反应蛋白(CRP)、白细胞(WBC)、血糖(BG)及体温(BT)的变化情况。结果术前1 d,两组CORT水平比较,差异无统计学意义(P>0.05);术后24 h、48 h和72 h,对照组CORT水平均高于观察组(P<0.05)。与术前1 d相比,术后24 h、48 h和72 h两组TNF-α、IL-6、CRP、WBC均明显上升(P<0.05);但两组间同时点比较,TNF-α、IL-6、CRP、WBC、BG、BT指标差异均无统计学意义(P>0.05)。观察组手术前后BG和BT的变化不明显(P>0.05)。对照组术后24 h BG及术后24 h、48 h BT较术前1 d明显上升(P<0.05);其余时点相比,差异无统计学意义(P>0.05)。结论与传统开放甲状腺手术相比,乳晕入路腔镜甲状腺手术并未增加对机体的创伤。
ObjectiveTo evaluate the degree of body trauma caused by endoscopic thyroidectomy via areola approach. MethodsA total of 60 patients with benign thyroid tumors were enrolled as subjects, among which, 30 cases receiving endoscopic thyroidectomy via areola approach were enrolled as observation group and 30 cases receiving conventional open surgery as control group. At 1 day before surgery, and at 24 h, 48 h and 72 h after operation, the variations of serum cortisol (CORT) , tumor necrosis factor alpha(TNF-α), interleukin 6(IL-6), C reactive protein(CRP), white blood cell(WBC), blood glucose(BG) and body temperature(BT) were statistical analyzed in both groups. ResultsThere was no significant difference in the serum CORT level at 1 day before surgery between the two groups(P>0.05); At 24 h, 48 h and 72 h after operation, the serum CORT levels of the control group were higher than those of the observation group(P<0.05). In both groups, the TNF-α,IL-6,CRP and WBC at 24 h, 48 h and 72 h after operation were significantly higher than those at 1 d before surgery(P<0.05); At the same time point, there were no significant differences in the TNF-α,IL-6,CRP,WBC,BG and BT between the two groups (P>0.05). In the observation group, no significant changes were found in the postoperative BG and BT levels compared to the preoperative levels(P>0.05). In the control group, the BG at 24 h after operation ,and BT at 24 h and 48 h after operation were significantly higher than those at 1 d before operation(P<0.05), but no statistical differences in the BG and BT were found among the rest time points (P>0.05). ConclusionCompared to conventional open thyroidectomy, endoscopic thyroidectomy via areola approach does not increase trauma to the body.
