目的探讨一次性海藻扩张棒在宫腔镜手术前宫颈预处理中的应用效果。方法选择因不规则子宫出血、子宫内膜息肉、不孕症、宫腔粘连、宫腔内异物残留等实施宫腔镜手术的患者108例,随机分为研究组和对照组各54例。研究组术前5 min给予常规消毒外阴、阴道后顺宫腔方向置入一次性海藻扩张棒进行宫颈预处理;对照组手术前一晚10:00 pm、术日晨6:00 am给予络合碘清洗外阴后于阴道后穹窿放置米索前列醇0.2 mg进行宫颈预处理。观察记录两组手术时间、术中出血量及宫颈扩张情况。结果研究组手术时间为(19.10 ±5.2)min,术中出血量为(25.10±8.5)mL,明显少于对照组的(30.20 ±6.8)min及(45.30±11.5)mL,差异均有统计学意义(P<0.05)。研究组宫颈扩张有效率为96.30%(52/54),高于对照组的81.48%(44/54),差异有统计学意义(P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论宫腔镜术前采用一次性海藻扩张棒进行宫颈预处理可使宫颈管软化、扩张良好,减少术中出血,缩短手术时间,有效提高了工作效率,值得临床推广。
ObjectiveTo investigate the efficacy of disposable laminaria dilator applied to cervical preconditioning before hysteroscopy. MethodsA total of 108 patients were enrolled and underwent hysteroscopic surgery due to irregular uterine bleeding, endometrial polyps, infertility, intrauterine adhesion or intrauterine foreign body residue, etc. . The patients were randomly divided into study group and control group, with 54 cases in each group.The study group received cervical preconditioning by placement of disposable laminaria dilator into uterine cavity after regular disinfection of vulva and vagina before 5 minutes of operation.The control group received cervical preconditioning by placement of misoprostol 0.2mg into posterior vaginal fornix after cleaning the vulva with iodine at the night before operation( 10 P.M) and in the morning of operation (6 A.M. ). The operative duration, intraoperative blood loss and condition of cervical dilation were recorded in both groups. ResultsThe operative duration and intraoperative blood loss in the study group [(19.10±5.2)min and (25.10±8.5)mL, respectively] were less than those in the control group [(30.20±6.8)min and(45.30±11.5)mL, respectively] (P<0.05). The effectiveness rate of cervical dilation in the study group was higher than that in the control group[96.30%(52/54)vs. 81.48%(44/54), P<0.05].There was no significant difference in the incident rate of complications between the two groups(P>0.05). ConclusionCervical preconditioning with disposable laminaria dilator before hysteroscopy can soften and expand the cervical canals, reduce intraoperative hemorrhage, shorten the operative duration, effectively improve the working efficiency, and is worthy of clinical promotion.
