彩超及CT在甲状腺癌诊断中的应用价值

目的探讨彩超在甲状腺癌(TC)诊断中的应用价值。方法对146例拟诊为TC患者的临床资料进行回顾性分析,所有患者均行彩色多普勒超声(简称彩超)和计算机断层扫描(CT)检查,以病理检查结果为“金标准”,比较彩超、CT与病理检查结果的符合率及诊断价值差异,观察两种诊断方法的关联性和优势性。结果彩超诊断敏感度、对乳头状TC及TC分型诊断总符合率均低于CT,对颈部淋巴结转移诊断符合率则高于CT(P<0.05)。彩超与CT诊断准确度、特异度比较,差异无统计学意义(P>0.05)。彩超、CT诊断关联性和优势性均具有统计学意义(P<0.05),彩超阳性检出率低于CT(P<0.05)。结论彩超诊断TC具有良好的准确度和特异度,与CT诊断关联性良好,对颈部淋巴结转移诊断较CT更具优势,且诊断无辐射,重复性好,于TC诊断有重要意义。
ObjectiveTo explore the value of color Doppler ultrasound in the diagnosis of thyroid carcinoma (TC). MethodsThe clinical data of 146 patients suspected as TC were retrospectively analyzed. All the patients underwent color Doppler ultrasound and computed tomography (CT). The result of pathological examination was taken as “gold standard”, then the coincidence rate and diagnostic value were compared between color Doppler ultrasound and CT. The correlation and diagnostic odds ratio between the two diagnostic methods were assessed. ResultsThe diagnostic sensitivity and the total coincidence rate for the diagnosis of papillary TC and TC were lower in color Doppler ultrasound compared to CT (P<0.05). But the coincidence rate for the diagnosis of cervical lymph node metastasis was higher in color Doppler ultrasound compared to CT (P<0.05). There were no significant differences in the accuracy and specificity between color Doppler ultrasound and CT (P>0.05). There were significant differences in the the correlation and diagnostic odds ratio between color Doppler ultrasound and CT (P<0.05). The positive detection rate of color Doppler ultrasound was lower than that of CT (P<0.05). ConclusionColor Doppler ultrasound obtains good diagnostic accuracy and specificity for TC, is well correlated with the diagnosis of CT. For the diagnosis of cervical lymph node metastasis, color Doppler ultrasound is superior to CT, has the advantages of nonradiation and good repeatability, and is significant for the diagnosis of TC.
