目的通过观察慢性心力衰竭(心衰)患者血清血管生成素-2(Ang-2)与BNP浓度、左室射血分数之间的关系,探讨Ang-2在心衰诊断中的价值。方法选取广西医科大学第一附属医院心内科收治的冠脉造影无异常且心脏彩超证实为扩张型心肌病的慢性心力衰竭患者16例为心衰组,另选取同期健康体检人群16例为对照组。收集两组临床化验资料及心脏彩超检查结果,检测血清标本Ang-2、BNP水平。结果两组年龄、性别、体重、身高、血压、血脂比较,差异无统计学意义(P>0.05)。心衰组BNP水平高于对照组,射血分数低于对照组,差异均有统计学意义(P<0.05)。心衰组血清Ang-2水平高于对照组,差异有统计学意义(P<0.05)。心衰组患者血清Ang-2水平与左室收缩末期内径呈正相关关系(r=0.281,P=0.026),与左室舒张末期内径呈正相关(r=0.256,P=0.003),与左室射血分数呈负相关关系(r=-0.280,P=0.030),与血清BNP浓度呈正相关关系(r=0.194,P=0.028)。结论随着心功能下降,慢性心力衰竭患者血清Ang-2的表达浓度明显增加,提示Ang-2和慢性心衰关系密切,可能为预防和治疗心衰提供新的方法与思路。
ObjectiveTo observe the relation of the serum angiogenin-2(Ang-2) with BNP concentration and left ventricular ejection fraction in the patients with chronic heart failure, thereby to evaluate the diagnostic value of Ang-2 for heart failure. MethodsSixteen chronic heart failure patients from the First Affiliated Hospital of Guangxi Medical University were enrolled as heart failure group. All of the patients were diagnosed as dilated cardiomyopathy by echocardiography and had no abnormal finding of coronary angiography. Sixteen healthy cases participating in physical examination during the same period were enrolled as control group. The data of clinical laboratory test and results of echocardiography were collected. The serum Ang-2 and BNP levels were detected. ResultsThere were no significant differences in the age, gender, weight, height, blood pressure and blood lipids between the two groups(P>0.05). The BNP level was higher and the ejection fraction was lower in the heart failure group compared to the control group(P<0.05). The Ang-2 level of the heart failure group was higher than that of the control group (P<0.05). In the heart failure group, the serum Ang-2 level positively correlated with left ventricular end-systolic diameter relation and left ventricular end-diastolic diameter (r=0.281, P=0.026;r=0.256, P=0.003) , negatively correlated with left ventricular ejection fraction (r=0.280, P=0.030), and positively correlated with serum BNP concentration (r=0.194, P=0.028). ConclusionIn the patients with chronic heart failure, the serum Ang-2 concentration significantly increases as the heart function declines, which suggests that Ang-2 is closely related to chronic heart failure. This may provide new method and idea for the prevention and treatment of heart failure.
