目的分析盆腔器官扭转的多排螺旋CT表现,探讨多排螺旋CT对盆腔器官扭转的诊断价值。方法回顾性分析24例经手术病理证实为盆腔器官扭转的多排螺旋CT表现,所有病例均行动脉期(30 s)、静脉期(60 s)及延时期(180 s)三期扫描。结果乙状结肠扭转1例,CT表现为乙状结肠转位及“鸟嘴征”;回肠扭转伴肠坏死2例,可见“咖啡豆征”及肠壁积气;肠系膜上动脉扭转伴小肠缺血4例, 或见“漩涡征”;卵巢黏液性囊腺瘤蒂扭转3例,其中左侧2例,右侧1例,见囊性肿块及分隔,分隔增厚,可见软组织样蒂,增强扫描呈轻度强化;右侧卵巢颗粒细胞瘤蒂扭转2例,表现为囊实性肿块;右侧卵巢良性囊性畸胎瘤蒂扭转3例,可见不对称增厚囊壁,扭转轻囊壁轻度增厚,扭转重囊壁明显增厚,增强扫描未见明显强化;右侧卵巢囊肿扭转5例,其中右侧3例,左侧2例,短期复查见囊肿增大,密度增高,囊肿发生位置移位;右侧输卵管积水伴扭转2例,可见增厚囊壁伴高密度出血;左侧隐睾恶变伴扭转1例,病变未见明显强化,内见积气,同侧精索及睾丸消失;左侧精索扭转1例,见精索增粗、模糊,同侧睾丸强化减弱。结论盆腔器官扭转的CT表现多样,“漩涡征”、“鸟嘴征”是肠扭转的特点;水肿增粗、强化减弱的“蒂”与原发肿瘤形成的囊实性“双肿块”提示卵巢囊腺瘤扭转;良性囊性畸胎瘤囊壁厚薄与扭转程度相关;同侧精索消失提示隐睾。多排螺旋CT对盆腔器官扭转的术前诊断具有重要意义。
ObjectiveTo analyze the multi-slice spiral CT findings of pelvic organ torsion and to discuss the value of multi-slice spiral CT applied to the diagnosis of pelvic organ torsion. MethodsThe multi-slice spiral CT findings of the patients pathologically diagnosed as pelvic organ torsion were retrospectively analyzed. All cases received three-phase scanning including arterial phase (30 seconds), venous phase (60 seconds) and delayed phase (180 seconds). ResultsOne case was diagnosed as sigmoid, and sigmoid transposition and ‘bird-beak’ appearance were observed in CT. Two cases were diagnosed as ileum volvulus complicated with bowel necrosis, ‘coffee-bean’ sign and pneumatosis intestinalis were observed in CT. Four cases were diagnosed as superior mesenteric artery volvulus complicated with intestinal ischemia, ‘whirlpool’ sign was observed in CT. Three cases were diagnosed as pedicle torsion of ovarian mucinous cystadenoma including 2 cases located on left side and 1 case on right side, cystic mass and septum, thickened septum, soft tissue pedicle were observed in CT and lightly intensification was observed when enhancement; Two cases were diagnosed as pedicle torsion of right ovarian granulosa cell tumor, cystic solid mass was observed in CT. Three cases were diagnosed as pedicle torsion of right ovarian benign cystic teratoma, asymmetrical thickened cystic wall, slight thickening cystic wall in mild torsion, obvious thickening cystic wall in severe torsion were observed in CT, and no obvious intensification was observed when enhancement. Five cases were diagnosed as ovarian cyst torsion including 3 cases located on right side and 2 cases on left side, enlarged cyst, increased density, and shifting cyst were observed in short-term CT review. Two cases were diagnosed as hydrosalpinx with torsion, thickened cystic wall and high-density hemorrhage were observed in CT. One case was diagnosed as left cryptorchidism with malignant transformation and torsion, no obvious intensification was found but pneumatosis and disappeared ipsilateral spermatic cord and testicle were observed in CT. One case was diagnosed as left spermatic cord torsion, thickened and indistinct spermatic cord and ipsilateral testicle with weakened intensification were observed in CT. ConclusionThe CT findings of pelvic organ torsion is various. Volvulusis is characterized by ‘whirlpool’ sign and ‘bird-beak’ appearance. Enlarged pedicle with edema and weakened intensification and cystic-solid ‘double mass’ originated in the primary tumor indicated ovarian cystadenoma torsion.The cystic wall thickness of benign cystic teratoma is correlated to the degree of torsion. The disappearance of ipsilateral spermatic cord indicated cryptorchidism. Multi-slice spiral CT is significant for the preoperative diagnosis of pelvic organ torsion.
