【摘要】目的探讨引起经皮穿刺椎体成形术(PVP术)后残余腰痛的相关危险因素。方法选择在我科住院并行PVP术的骨质疏松性椎体压缩性骨折患者120例为观察对象。收集患者年龄、性别、诱因(有无外伤)、病程、病史(既往有无腰痛)、体重指数、骨密度(BMD)、椎体高度、术前VAS评分、术后椎体高度恢复情况、骨水泥注入量、骨水泥渗漏等相关因素。观察PVP术后发生残余痛的情况,根据有无残余痛分为观察组与对照组,并对比上述各因素。结果两组性别、年龄、病程、术前椎体前缘高度、术前椎体中间高度、骨水泥注入量、骨水泥渗漏率、术前VAS评分比较,差异无统计学意义(P>0.05);两种诱因、病史、体重指数、BMD、术后椎体前缘高度、术后椎体中间高度、术后椎体前缘高度恢复、术后椎体中间高度恢复比较,差异有统计学意义(P<0.05)。对相关因素进行Logistic回归分析,结果表明体重指数、BMD、术后椎体中间高度恢复、诱因是引起PVP术后残余腰痛的危险因素。结论低体重指数、低骨密度、椎体中间高度恢复不良、外伤史是椎体成形术后残余痛发生的主要因素。
【Abstract】 ObjectiveTo explore the risk factors related to residual back pain after percutaneous vertebroplasty (PVP). MethodsA total of 120 patients with osteoporotic vertebral compression fractures undergoing PVP were enrolled. The relevant factors were collected including patient′s age and gender, precipitating factor (with or without trauma history), disease duration, history (with or without back pain previously), body mass index,bone mineral density,(BMD), vertebral height, preoperative Visual Analogue Scale(VAS), postoperative improvement of vertebral height,injection volume and leakage of bone cement and etc. The incidence of residual back pain after PVP was observed. The patients were divided into observation group and control group according to the incidence of residual back pain. ResultsThere were no significant differences in the gender, age, disease duration, preoperative anterior edge and medium heights of vertebral body, injection volume of bone cement, rate of bone cement leakage and preoperative VAS between the two groups (P>0.05).There were significant differences in the precipitating factor, history, body mass index, BMD, postoperative anterior edge and medium heights of vertebral body, postoperative improvement of anterior edge and medium heights of vertebral body between the two groups (P<0.05) . Relevant factors were enrolled in logistic regression analysis, and the results showed that body mass index,BMD , the medium height improvement of vertebral body and precipitating factor were the risk factors for residual back pain after PVP. ConclusionLow body mass index, reduced bone mineral density, poor recovery of medium height of vertebral body,injury history are the main causes of residual back pain after vertebroplasty.
