目的观察自制小型牵引外固定器在胫骨骨折髓内钉内固定术中的应用效果。方法40例胫骨干骨折患者,随机分为观察组和对照组,每组20例。观察组接受交锁髓内钉内固定治疗,术中应用自制小型牵引外固定器行患肢固定及牵引复位;对照组单纯交锁髓内钉内固定治疗。比较两组患者手术时间、术中出血量、骨折愈合时间、临床疗效等情况。结果40例患者均获随访,随访时间为4~18个月,平均12个月。观察组手术时间、术中出血、骨折愈合时间均少于对照组,两组比较,差异有统计学意义(P<0.05)。两组临床疗效比较,差异无统计学意义(P>0.05)。结论在胫骨骨折交锁髓内钉内固定术中,小型牵引外固定器可起到固定小腿、持续牵引的作用,既减轻助手辛苦,又可避免在复位过程中由于经常变化位置而造成骨折周围软组织的二次损伤,可明显缩短手术时间,减少术中出血量及骨折愈合时间,值得临床推广应用。
ObjectiveTo observe the application effect of self-made miniature external fixator in the treatment of tibial fracture with intramedullary nail fixation. MethodA total of 40 patients with tibial fractures were divided into observation group and control group, 20 cases in each group. The observation group were treated with intramedullary nail fixation, using the self-made miniature traction eternal fixator to fix and tow the injured limb. The control group were treated with intramedullary nail fixing. The operation time, intraoperative blood loss, fracture healing time and clinical efficacy were compared between the two groups. ResultsBoth of the 40 cases were followed up for an average of 12 months (range from 4 to 18 months). The operation time, intraoperative blood loss, fracture healing time of the observation group were less than those of the control group, the difference between the two groups was statistically significant (P<0.05). There was no significant difference in clinical efficacy between the two groups (P>0.05). ConclusionThe self-made miniature traction eternal fixator can fix and tow the tibial, reduce the assistant,s hard, avoid soft tissue injury, shorten operation time,reduce the surgical bleeding and the fracture healing time. The self-made miniature traction eternal fixator is worthy to be promoted.
