目的探讨DHS内固定与人工股骨头置换术治疗老年股骨粗隆间骨折的临床效果。方法将接受股骨粗隆间骨折手术治疗的90例患者根据入院时间将患者分为观察组和对照组，每组45例，观察组采用人工骨头置换术进行治疗，对照组采用DHS内固定进行治疗。观察分析两组患者的治疗效果。结果观察组手术时间、出血量与对照组比较，差异无统计学意义（P＞0.05），而住院天数、负重训练开始时间明显短于对照组（P＜0.05）。观察组患者HSS评分优良率为80.0%（36/45），明显高于对照组60.0%（27/45），两组比较，差异有统计学意义（P＜0.05）。经治疗后，观察组治疗有效率为84.4%（38/45），明显高于对照组60.0%（27/45），两组比较，差异有统计学意义（P＜0.05）。观察组并发症发生率为6.6%（3/45），明显低于对照组的22.2%（10/45）。结论对于股骨粗隆间骨折老年患者采用人工股骨头置换进行治疗的效果优于DHS内固定治疗，并发症少，对于改善患者生活质量有着重要意义，值得广泛推广。

ObjectiveTo evaluate the clinical effect of DHS internal fixation and artificaial femoral head arthrolasty in the treatment of intertrochanteric fracture in elderly patients. MethodsA total of 90 patients with intertrochanteric fracture were were divided into observation group and control group according to the time of admission. The observation group were treated with artificaial femoral head arthrolasty, the control group were treated with DHS internal fixation. To observe and analyze the treatment effect of the two groups. ResultsCompared with the control group, the operation time and blood loss of the observation group were not statistically significant (P＞0.05); the length of hospital stay and the start time of weight bearing training were significantly shorter than those of the control group (P＜0.05).The excellent rate of HSS score in observation group was 80.0%(36/45), significantly higher than 60.0%(27/45) in the control group(P＜0.05). After treatment, the observation group effective rate was 84.4%(38/45), significantly higher than 60.0%(27/45) of the control group(P＜0.05). Incidence rate of complication in observation group was 6.6%(3/45), significantly lower than 22.2%(10/45) in the control group(P＜0.05). ConclusionsIntertrochanteric fractures in elderly patients with hemiarthroplasty for treatment is better than the DHS fixation treatment, and fewer complications, improve quality of life for patients is of great significance,it should be popularized.
