目的探讨骶韧带-耻骨阴道肌交叉缝合治疗前盆腔脱垂的有效性及安全性。方法将因重度子宫脱垂合并前盆腔脱垂的患者30例,进行阴式全子宫切除+骶韧带-耻骨阴道肌交叉缝合(合并张力性尿失禁行尿道后韧带折叠术)+阴道残端右侧骶棘韧带固定术及后壁修补手术,记录手术时间、出血量、术中及术后并发症;采用POP-Q评估手术治疗效果,采用盆底功能障碍性疾病症状问卷(PFDI-20)、盆底疾病生命质量影响问卷(PFIQ-7)比较患者术前、术后生活质量评分。结果①平均手术时间(95.3±22.5)min,平均出血量(210.3±70.8)mL,术后留置尿管时间(3.2±1.6)d,平均住院时间(6.2±2.3)d,术中均无膀胱、直肠及坐骨神经损伤发生。②对患者随访12~20个月,中位随访时间为14个月,患者术后1年时Aa、Ba、C、Bp和Ap点长度均较术前显著改善,差异有统计学意义(P<0.01)。患者PFIQ-7评分,术后(32.40±6.53)分较术前(91.43±13.86)分下降,差异有统计学意义(P<0.01),PFDI-20评分,术后(27.16±8.11)分较术前(93.00±14.55)分下降,手术前后比较,差异有统计学意义(P<0.01)。结论骶韧带-耻骨阴道肌交叉缝合可有效恢复前盆腔脏器解剖位置,改善患者脱垂症状及生活质量,术中膀胱损伤小,术后无网片暴露,值得临床推广应用。
ObjectiveTo explore the efficacy and safety of uterosacral ligament and pubovaginalis in the treatment of anterior pelvic organ prolapse. MethodsA total of 30 patients with severe uterine prolapse and anterior pelvic organ prolapse,were treated by transvaginal panhysterectomy,suture of uterosacral ligament and pubovaginalis,sacrospinous ligament fixation and posterior wall neoplasty. Operation time, blood loss, intraoperative and postoperative complications were recorded. The surgical treatment effect was evaluate by POP-Q. And preoperative and postoperative quality of life score was evaluated by Pelvic Floor Distress Inventory-20 (PFDI-20), Pelvic Floor Impact Questionnaire-7 (PFIQ-7). Results①The average operation time was (95.3±22.5)min,the average bleeding volume was (210.3±70.8)mL,the time of urinary catheter was (3.2±1.6)d,the average length of hospital stay was (6.2±2.3)d. No bladder injury,rectum injury and sciatic nerve injury were observed. ②All the patients were followed up for 12-20 months, the median follow-up time was 14 months. After postoperative one year, the length of Aa、Ba、C、Bp and Ap were all better than those before surgery,the difference had statistical significance(P<0.01). The postoperative scores of PFDI-20 and PFIQ-7 were decreased compared with those before sugery(32.40±6.53 VS 91.43±13.86; 27.16±8.11 VS 93.00±14.55),the difference had statistical significance(P<0.01). ConclusionSuture of sacral ligament and pubovaginalis can not only effectively recover patients’ antomical structure, but also can significantly improve patients′ quality of life. With small intraoperative bladder injury, less postoperative mesh exposure, the operation is valuable to be used in clinical application.
