目的探讨腹腔镜下应用电钩联合超声刀精细解剖胆囊三角技术在腹腔镜胆囊切除术(LC)中临床应用价值。方法76例胆囊结石患者,均采用电钩联合超声刀精细解剖胆囊三角技术行腹腔镜胆囊切除术(LC)。结果73例患者顺利完成LC手术。其中腹腔镜下胆囊切除(LC)联合阑尾切除(LA)6例,2例因Mirizzi综合征粘连严重胆囊三角解剖不清,1例因胆囊坏死,三管结构无法辨别中转开腹手术。手术时间35~96 min,平均49.5 min,术中出血量约6~70 mL,平均19.5 mL。术中未出现胆管损伤。5例需放置橡胶引流管引流。住院时间4~7d,平均4.5 d。术后76例患者随访3~9个月,平均7.6个月,均无胆瘘、胆管狭窄、肠粘连及腹腔脓肿等并发症发生。结论采用电钩联合超声刀精细解剖胆囊三角技术行腹腔镜胆囊切除术(LC),在准确辨清胆囊区域特别是胆囊三角区域的胆囊管、肝总管、胆总管三管局部解剖关系的前提下切除胆囊,创伤小,可有效预防胆道损伤,安全可行,具有较好临床应用价值价格。
ObjectiveTo investigate the clinical value of the application of laparoscopic electrical hook combined with ultrasonic knife in gallbladder triangle fine anatomy for laparoscopic cholecystectomy (LC). MethodsA total of 76 patients with gallstones were treated with electrical hook combined with ultrasonic knife to conduct fine anatomy in gallbladder triangle for LC. Results73 patients successfully underwent LC surgery. Among them, 6 cases conducted LC combined with laparoscopic appendectomy (LA), 2 cases had unclear severe gallbladder triangle anatomy due to Mirizzi syndrome caused adhesion, 1 case was transferred to laparotomy surgery due to cystic necrosis and undistinguished three duct structure. The operative time was 35 to 96 min,with an average of 49.5 min,the intraoperative blood loss was 6 to 70 mL, with an average of 19.5mL.No intraoperative bile duct injury occurred. Five cases conducted drainage by placing rubber drainage tube. the length of Hospital stay was 4-7 d, with an average of 4.5d,respectively. Postoperation follow-up performed for 3 to 9 months, with an average 7.6 months,and no complications such as bile leakage,biliary stricture or intraperitoneal abscess occurred. ConclusionThe electrical hook combined with ultrasonic knife was used to conduct fine anatomy in gallbladder triangle for LC, which can accurately discern gallbladder region, especially the three duct local anatomical relations of cystic duct, common hepatic duct and common bile duct in gallbladder triangle region, thus the wound was small in gallbladder removal, can effectively prevent bile duct injury, is safe and feasible, and has good clinical values.
