目的探讨应用持续加压空心螺纹钉治疗明显移位型(Ⅲ、Ⅳ型)中青年股骨颈骨折的临床疗效。方法将中青年股骨颈骨折患者110例分为A、B两组,A组58例采用持续加压空心螺纹钉内固定治疗,B组52例采用单纯空心螺钉内固定治疗,比较两组疗效。结果随访19~52个月。A组愈合时间为85~126 d,平均愈合时间为(102±18)d,骨折不愈合2例,股骨头坏死4例;B组愈合时间为130~176 d,平均愈合时间为(150±28)d,骨不愈合9例,股骨头坏死12例;两组的手术时间、X线暴露时间、末次随访时Harris功能评分比较,差异无统计学意义(P>0.05);A组骨折愈合时间、并发症发生率明显低于B组,两组比较,差异有统计学意义(P<0.05)。结论应用持续加压空心螺纹钉治疗中青年股骨颈骨折具有再次手术几率低、骨折不愈合率和骨坏死发生率低的优点。
ObjectiveTo explore the efficacy of continuous pressurized hollow screw fixation in treatment of young and middle-aged patients with significant shift type (Ⅲ, Ⅳ type) femoral neck fracture. MethodsA total of 110 young and middle-aged patients with femoral neck fracture were divided into group A and group B. Fifty-eight cases in group A were treated with continuous pressurized hollow screw fixation, fifty-two cases of group B were treated with hollow screw fixation. The efficacy of the two groups were compared. ResultsPatients were followed up for 19 to 52 months. Healing time of group A was 85-126 d, the average healing time was (102±18) d, fracture nonunion occurred in two patients, femoral head necrosis in four cases; Healing time of group B was 130-176 d, the average healing time was (150±28) d, fracture nonunion occurred in 9 cases, 12 cases with femoral head necrosis. There were no significant difference in the operation time, X-ray exposure time, and the Harris function score at the time of last follow-up between the two groups (P>0.05); Fracture healing time, concurrent disease incidence of group A were significantly lower than that of group B, the differences were statistically significant (P<0.05). ConclusionIt has the advantages of low reoperation rate, low fracture nonunion rate and low incidence of bone necrosis using continuous pressurized hollow screw fixation in treatment of femoral neck fracture of young and middle-aged patients.
