目的探讨一次性手术取尽复杂性输尿管上段及下段结石的有效办法。方法16例复杂性输尿管结石患者均在全麻下先行后腹腔镜输尿管上段或肾盂结石切开取石，顺利置入DJ管后，再取截石位在输尿管硬镜下气压弹道将下段结石完全击碎排出，将DJ管尾端拖入膀胱盘旋一周。结果16例患者结石均无残留，手术时间（120±40）min。2个月后复查B超，肾集合系统分离较手术前明显好转，未见强回声结石影。结论后腹腔镜联合输尿管硬镜治疗复杂性输尿管结石能一次性取出结石，减少患者痛苦及减轻经济负担，是较好的手术方法之一。

ObjectiveTo explore the effective way to remove complex upper and lower ureteral calculi by one operation. MethodsSixteen cases of complex ureteral calculi were performed with retroperitoneoscopic upper ureterolithotomy or pyelolithotomy,and were implanted with double J stent, followed by pneumatic ballistic lithotripsy via rigid nephroscope with D J tube tail drugged into bladder and curved in a circle. ResultsNo calculi residual was observed in all 16 patients. The operation time was (120 ±40) minutes. Bultrasound image 2 months later showed separation of renal collecting system improved markedly without stone of strong echo. ConclusionsRetroperitoneoscopy in combination with rigid ureteroscope can remove the stone in one time, thus relief patient ′s pain and economic burden, indicating it as an excellent surgical method to treat complex ureteral calculi
