目的探讨微创双切口结合双带袢钢板固定技术治疗完全型肩锁关节脱位的应用价值。方法完全型肩锁关节脱位患者54例,根据不同的术式分为对照组(锁骨钩钢板固定)和观察组(微创双切口结合双带袢钢板固定)各27例,比较两组切口长度、出血量、恢复工作时间和随访6个月复位丢失情况,随访1年后作Constant评分及疗效。结果与对照组比较,观察组失血少,切口小,愈合快;观察组患肢外展及前屈上举可活动度均大于对照组,复位丢失更少;观察组Constant总分低于对照组(P<0.05)。结论微创双切口结合双带袢钢板内固定术后基本无肩峰碰撞,利于康复锻炼;且手术切口小,出血量少,术后肩关节活动情况良好,无需二次手术。
ObjectiveTo explore the efficacy of two-incision minimally invasive technique combined with double belt ban steel plate in the treatment of complete acromioclavicular joint dislocation. Methods54 patients with complete acromioclavicular joint dislocation were divided into control group (clavicular hook plate) and observation group (two-incision minimally invasive plate fixation combined with double belt loop),according to the surgical procedures,27 cases in each group.The incision length, blood loss, recovery time, up 6 month loss of reduction,1-year follow Constant scores and clinical effect were compared between the two groups. ResultsCompared with the control group, the blood loss of observation group was less, incision was smaller, healing faster.The observation group limb abduction and flexion lift movable degree were higher than the control group, with less loss of reduction.The total score of Constant in the observation group was lower than that in the control group(P<0.05).ConclusionAfter two-incision minimally invasive technique combined with double belt ban steel plate treatment,there is almost no shoulder collision, conducive to rehabilitation exercise. With smaller operation incision, less amount of bleeding and better shoulder joint activity, the second operation is no need.
