目的 研究心房颤动(atrial fibrillation,AF)患者右房-冠状窦电传导的特点。方法 房颤组为阵发性房颤患者23例,对照组为阵发性室上性心动过速或室性早搏患者19例。应用固定程序S1S1 500、400、350、300、280 ms起搏低位右心房(LRA),测量低位右心房到冠状窦CS近端、CS远端的传导时间。结果 对照组和房颤组的低位右心房到冠状窦近端、远端的传导时间有差异,随着起搏时间的递减S1S1 500 ms~400 ms尤以显著。两组比较,差异有统计学意义(P<0.05)。结论 冠状窦心房交界处存在传导电延缓,可能是心房颤动的发生机制。
Objective To study the characteristics of electrical conduction between right atrium and coronary sinus in patients with atrial fibrillation (AF). Methods A total of 23 patients with paroxysmal atrial fibrillation were enrolled in the AF group, and 19 patients with paroxysmal supraventricular tachycardia or ventricular premature beats were enrolled in the control group. Pacing was performed at low right atrium (LRA) at cycle length of 500, 400, 350, 300 and 280 ms by fixed program S1S1. Conduction time from LRA to the proximal coronary sinus (CS) and distal CS was measured. Results There was a difference in the conduction time from LRA to the proximal CS and distal CS between the AF group and the control group, especially when pacing cycle length descended to S1S1 500 to 400 ms. There was a statistical difference between the two groups (P<0.05). Conclusions Delayed electrical conduction occurs in the junction of the coronary sinus and atrium, and it may be involved in the mechanism of atrial fibrillation.
