目的 探讨超声引导下8gauge麦默通(Mammotome,MMT)真空辅助微创旋切系统在乳腺多发病灶诊断和治疗中的临床应用价值。方法 在超声引导下进行8gauge MMT微创旋切术的80例患者,根据病灶数目分为高发组42例(单侧乳房病灶≥7个)和对照组38例(3个≤单侧乳房病灶<7个),比较两组患者的手术时间、出血量及术后并发症。结果 全部病灶均经8gauge MMT微创旋切术切除,高发组在手术时间、出血量等方面均高于对照组,差异有统计学意义(P<0.01)。术后6个月随访,两组患者手术切口均愈合良好,乳房无变形。结论 8gauge麦默通微创旋切系统切除乳腺多发病灶具有简单安全、微创美观等特点,尤其适用于诊治双乳多发隐匿性病灶,随着切除病灶数目的增多其并发症的发生率也相应增高。
Objective To investigate the clinical value of ultrasoundguided 8gauge vacuumassisted Mammotome (MMT) system for the diagnosis and treatment of multiple breast lesions. Methods Eighty patients underwent ultrasoundguided 8gauge MMT minimally invasive resection. The patients were divided into high occurrence group (the sum of unilateral lesions≥7; n=42) and control group (3≤the sum of unilateral lesions<7; n=38) according to the sum of lesions. The operative duration, blood loss and postoperative complications were compared between the two groups. Results All breast lesions were minimally invasively resected by the 8gauge MMT system in all cases. The operative duration was longer and the blood loss was more in the high occurrence group compared with the control group (P<0.01). During the postoperative followup of 6 months, good incision healing was observed in both groups, and no breast deformity occurred in any cases. Conclusion 8gauge MMT minimally invasive surgery is a simple, safe and minimally invasive approach for resection of multiple breast lesions, especially for bilateral multiple nonpalpable lesions. However, the incidence of complications will rise while the number of removed lesions is increased.【=
