目的 探讨直视下椎体成形术联合椎弓根内固定治疗脊柱转移癌的临床效果。方法 对脊柱转移癌22例患者行直视下经皮椎弓根穿刺椎体成形术联合椎弓根内固定术。观察患者手术前及术后1周、1个月、6个月、12个月的VAS评分、侧位X片矢状面的Cobb′s角度以及椎体前、中、后缘的高度数值，采用术前术后自身对照进行统计学分析。结果 VAS评分术后较术前明显降低；脊柱后凸角度（Cobb′s角度）较术前减少；椎体间高度较术前增加。结论 直视下椎体成形术联合椎弓根内固定能有效并持久的缓解脊柱转移癌引起的腰背疼痛，增加椎体强度，恢复并维持椎体高度及矫正后凸畸形。

Objective To discuss the clinic efficacy of euthyphoria percutaneous vertebroplasty combine with posterior approach decompression and fusion for spinal metastases tumor. Methord 22 cases suffering from spinal metastases tumor from January 2010 to December 2013 underwent euthyphoria percutaneous vertebroplasty combine with posterior approach decompression and fusion. All the cases were followed up and the VAS scores of back pain, the degree of spinal kyphosis (Cobb′s angle), the height of pathological vertebral body in lateral Xray film at postoperative one week,1st, 6th, 12th month were measured and analyzed. Results Postoperative VAS score of back pain and the degree of spinal kyphosis (Cobb′s angle) were significantly decreased, while the height of pathological vertebral body were significantly increased. Conclusion Eeuthyphoria percutaneous vertebroplasty combine with posterior approach decompression and fusion provides immediate and longterm back pain relief, recovers the height of pathological vertebral body, corrects spinal kyphosis and contributes to spinal stabilization.
