目的探讨锁骨钩钢板配合早期功能锻炼治疗锁骨远端骨折及肩锁关节脱位的临床疗效。方法对收住院的61例锁骨远端骨折及肩锁关节脱位患者,采用锁骨钩钢板配合早期功能锻炼治疗32例,克氏针张力带钢丝固定治疗29例,比较两组患者术后满意率及并发症的发生情况,按照Constant评分法评定术后肩关节外展上举评分及优良率。结果 61例患者均获得6~36个月随访,术后12个月两组优良率差别无统计学意义,锁骨钩钢板配合早期功能锻炼患者的肩关节外展上举范围优于克氏针张力带钢丝患者。结论锁骨钩钢板配合早期功能锻炼内固定稳定性强,肩关节外展上举范围较克氏针张力带钢丝内固定有明显的改善,而克氏针张力带钢丝取材方便,适于基层医院推广使用。

Objective To explore the clinical efectiveness of clavicular hook plate combined with early function exercise in the treatment of distal clavicle fracture complicated with acromioelavlcular joint dislocation．Methods 61 cases of distal clavicle fracture complicated with acromioclavicular joint dislocation were admitted and treated with clavicular hook plate with early function exercise(32 cases)，or tension band wire fixation(29 cases)．Postoperative satisfaction rate and complication incidence of the two groups were compared，and abduction and raise score and fineness rate of shoulder joint Was evaluated in accordance with the Constant Score Scale．Results 61 patients were followed up for 6 tO 36 months．There Was no significant diference in the fineness rate between the two groups 12 months after operation，but the abduction and raise SCOre of shoulder joint and the fineness rate were superior in group treated with clavicular hook plate combined with early function exercise than with tension band wire fixation．Conclusions Clavicular hook plate combined with early function ex— ereise results in stable internal fixation and improvement in abduction and raise range of shoulder joint，whereas tension band wire fixation is shown to be more available in material and suitable to the primary hospita1．
