目的探讨64排128层CT血管成像(CTA)诊断颅内动脉开窗畸形的诊断价值,了解颅内动脉开窗畸形CTA表现、检出率以及好发部位。方法对经64排128层CTA诊断为颅内动脉开窗畸形的24例患者的影像学资料进行回顾性分析,分析畸形的部位、类型及其合并其他颅内血管性病变。结果 327例中,颅内动脉开窗畸形24例(7.3%),共计25处;10例位于椎动脉,8例位于基底动脉,4例位于大脑前-前交通动脉区;大脑中动脉1例;1例为前交通区与基底动脉2处多发。裂隙型10处,凸透镜型13处,重复型2处。3例合并颅内动脉瘤,1例同时合并AVM及动脉瘤。结论 64排128层CTA能够无创地清楚显示开窗畸形的位置、形态及合并颅内其他血管性病变,可以作为颅内动脉开窗畸形首选的影像检查方法。
Objective To explore the diagnostic value of 64 slices 128 layers CT angiography( CTA) in intracranial arteries fenestration,its the imaging characteristic,the frequency and common location of intracranial arterial fenestration. Methods The imaging data of 24 patients with intracranial arteries fenestration diagnosed by 64 slices 128 layers CT angiography were retrospectively analyzed. The variation location,type and other coexisting vascular anomalies were analyzed. Results Of 327 patients who was underwent the CTA,24 cases( 25 locations) were detected with fenestrations,with 10 cases in vertebral artery,8 in basilar artery,4 in anterior communicating artery region,1 in middle cerebral artery,and 1 in both anterior communicating artery region and basilar artery. 10 were categorized as slit-pattern,13 convex-lens-pattern,and 2 duplication-pattern. 3 were complicated with aneurysms,and 1 with both aneurysms and AVM.
Conclusion 64-slices 128 layers CTA is an accurate method for demonstrating the location,type of intracranial arteries fenestration and other coexisting intracranial arterial abnormalities,suggesting it as an optimum noninvasive imaging tool for the diagnosis of intracranial arteries fenestration.
