目的分析应用单丝聚丙烯补片修补老年腹壁巨大切口疝的治疗效果。方法对36例老年巨大切口疝使用单丝聚丙烯补片修补,其中肌鞘前修补（onlay）12例,肌肉与肌肉间修补（inlay）5例,肌肉后腹膜前修补（sublay）19例,平均随访24个月。结果无复发病例,无切口感染,6例有皮下积液,均为onlay法,9例有不同程度异物感,但不妨碍日常生活。结论老年腹壁巨大切口疝应用单丝聚丙烯补片无张力修补,可取得满意疗效,采用妥善的术中、术后处理方法,可减少术后并发症的发生

Objective To analyze the application of monofilament polypropylene patch in the treatment of giant abdominal incisional hernia in the aged.Methods Monofilament polypropylene patch was used in 36 cases of senile giant incisional hernia.Among them,12 cases were fixed before the muscle sheath,5 between the muscle,and 19 before peritoneum and after muscle.All patients were followed up for an average of 24 months.Results Subcutaneous fluid was observed in 6 cases（all fixed before muscle sheath）,and foreign body sensation in varying degrees in 9.No recurrence,or wound infection occurred in all patients.Conclusions Monofilament polypropylene tension-free repair patch used in elderly giant abdominal incisional hernia can achieve a satisfactory effect,and adoption of proper intra and postoperative treatment can reduce postoperative complications.
